Medicare Case Management Demonstration Project

Mercy Medical Center-North Iowa, affiliated hospitals and Mercy Clinics have been selected to participate in a Medicare funded research project at Mercy, which will ultimately benefit you. This Medicare Case Management Demonstration Project is one of only 15 across the country.

Purpose
The primary purpose of this research project is to improve patient health, improve access to health care services and decrease the cost to Medicare. More than 600 Medicare beneficiaries are needed to participate locally.

How does the project work?
One half of those participating will receive current Medicare health care services as usual. The other half will receive case management along with the usual Medicare health care services. Medicare will compare the cost of receiving care with case management to the cost of receiving care without case management.

What is case management?
A nurse, nurse practitioner or social worker will work with you and your doctor to manage your health care needs. The case manager will identify your health care needs early on to avoid serious complications and to address the problems or barriers that keep you from getting the care needed, thereby avoiding unnecessary hospitalizations.

What is the cost?
There is no cost to you for participating.

An appointment will be scheduled for a case manager to visit with you and thoroughly explain the project before you give your consent.
