Diabetes Education
Inpatient and Home Care Diabetes Education
The diabetes education program includes an inpatient component. A nurse diabetes educator meets with hospitalized patients who have just been diagnosed or whose diabetes may be out of control. She provides the patient with the required survival skills until they can attend the outpatient classes. She also works with Mercy Home Care, acting as a case manager for the complex case patients who struggle with controlling their diabetes and is a resource to the home care nurses.
